I, the undersigned, wish to voluntarily participate in the Krav Maga class provided by your local Hillel (“the Organization”). In consideration of being allowed to participate in this activity, I, for myself and on behalf of my heirs, executors, administrators, and assigns, hereby knowingly and voluntarily enter this Waiver and Release of Liability.
- Assumption of Risk: I understand that Krav Maga is a physical activity that involves various movements, exercises, and techniques, and that participation in this activity carries certain inherent risks of injury. I acknowledge that I have been informed of these risks and am participating voluntarily.
- Release of Liability: I hereby release, discharge, and hold harmless the Organization, its employees, instructors, agents, and representatives from any and all claims, actions, or liabilities arising out of my participation in the Krav Maga class, including but not limited to personal injury, property damage, or any other harm that may occur during or as a result of my participation.
- Medical Authorization: In the event of an emergency, I authorize the Organization and its representatives to obtain necessary medical treatment for me and to release any medical information required for treatment.
- Photography and Likeness: I grant permission for the Organization to use any photographs, videos, or recordings of me taken during the Krav Maga class for promotional and educational purposes. Additionally, I consent to being contacted by Masa Israel Journey regarding opportunities and updates related to their programs and activities.
