Before you complete The MASA online application form, please be sure you have the following:

• Name of the program to which you are applying;
• Your contact information, including a current e-mail address to which MASA will send the print version of the application form;
• If you are also applying for a needs-based scholarship, please be sure to have your parents' current financial information at hand (or yours if you are applying as an independent) in order to complete the needs-based portion of the application. 

Complete and submit the online application.  A printable .pdf version will be sent to the e-mail address you entered in the application form within a few days.  Print the form, sign and date it and submit it to the Program Organizer whose name appears on the form.*  If you are also applying for a needs-based scholarship, please attach the appropriate financial documentation.
